I am registering for a Program of the Lawrence Conservatory during the program year (August-June). I acknowledge that the Program is voluntary and may involve certain risks, including those described below.

I understand that the Program involves, among other activities, rehearsing, performing, engaging in recreation, and, if applicable, traveling off campus. I acknowledge that these activities present certain inherent risks beyond Lawrence University’s control.

I choose to participate in the Program understanding its associated risks, and I knowingly and voluntarily assume all risks of injury, illness, death, or damage to or loss of my property that may occur while rehearsing, performing, traveling, engaging in recreation, or any activities arising out of or related to the Program.

My participation in the Program is voluntary, and I understand that I may discontinue my participation at any time in light of the risks I am assuming.

I hereby forever release Lawrence University, its trustees, officers, employees, and agents (the “Released Parties”), from liability for any negligent act or omission arising from my participation in the Program, including related costs, damages, claims, or assertions to which I or my family, estate, or heirs may claim against the Released Parties.

This Assumption of Risk and Release of Liability shall be construed in accordance with the laws of the State of Wisconsin, which shall be the forum for any lawsuits filed under or incident to this Release or the Program.

Acknowledgement

By checking this box, I, parent and/or guardian for the Participant, hereby agree on behalf of my minor child to the terms above, and agree to indemnify and hold harmless, Lawrence University from any loss or liability they should incur in any way because of my child’s participation.

I am a Participant in a Lawrence University camp. I give permission to Lawrence University, and its partners and affiliates (collectively, “Lawrence”) to use photographs and/or video of my image (collectively “Image”) for marketing and promotion, including, but not limited to, Lawrence websites, newsletters, social media, and advertisements (including commercial endeavors).

In consideration for the benefit I received by attending the Lawrence University camp and for other good and valuable consideration herein acknowledged as received, I hereby grant to Lawrence the perpetual, irrevocable and unrestricted right and permission to copyright, in its own name or otherwise, and use, re-use, publish, and re-publish my Image without restriction as to changes or alterations, and in any and all media now or hereafter known.


I also consent to the use of any printed matter in conjunction therewith. I hereby waive any right that I may have to inspect or approve the finished product and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.

By checking this box, I, parent and/or guardian for the Participant, do hereby release, acquit and forever discharge Lawrence, its officers and employees from any and all demands, cause of action and/or judgments of whatsoever nature of character, past or future, known or unknown, whether in contract or in tort, for or damages of any kind or nature arising out of, in any way, the use of my Image for Lawrence business.
