This form is required for all faculty and staff seeking external funding. It will be reviewed by the Provost and/or relevant Cabinet member(s) for approval before you can proceed with your application.

Submitting this form helps Lawrence track and support your application, but it does not commit you to submitting a grant or guarantee cost-sharing from Lawrence funds. If some details are uncertain, please provide your best estimates.
